With the day-to-day changes surrounding COVID-19, we do not know how much longer we will be open to the public during the next several weeks. What that being said, if you are a contact lens wearer and currently have less than a 3-month supply remaining, we recommend that you place an order now and we will ship your contacts directly to you with no charge for shipping.

We will extend contact lens prescription expiration dates on a case-by-case basis.

We are opening soon!

We’re exciting to announce that we will be reopening Monday, May 4th. In order to reopen safely, we will be following the requirements from the CDC and the Texas Board of Optometry. Please be patient as we work to ensure everyone is seen as safely and quickly as possible.

Listed below are changes to keep our staff and patients safe:

1. All patients will be required to wear a mask at all times while in the office. If you do not have one, we will provide you with one.
2. Patients should not have more than one person accompany them during their visit.
3. New patients should fill out paperwork online. It can be found on our website (www.creeksidefamilyeyecare.com) under patient forms. There are printable and online forms available to submit.
4. Our doors will remain locked. When you arrive, please call or contact our office to let us know you are here to check in. We have a virtual waiting room/check-in, please wait in your car and will let you know when to come in. We will ask everyone to sanitize or wash hands upon entering the office. We will be conducting temperature checks on all patients. Anyone with 100.4 degrees or higher will be asked to reschedule.
5. We ask that you please strictly adhere to social distancing in our office.
6. If you have been sick within the past 3 weeks, traveled outside the U.S., or have been in contact with COVID positive individuals, we kindly ask you to reschedule your appointment. We will have pre-screening questionnaires sent before each appointment.
7. We have tele-health visits available for your safety.
8. All staff will be screened and wearing masks at all times.
9. The hours will be limited and appointments are spread out to reduce contact with other patients.
10. Pickups/dispensing will be done by appointment only and curbside.
11. All surfaces/equipment will be disinfected between every patient.
12. All frames will be disinfected after trying on, please do not put the frames back on the boards.
